Family Care Community Partnership (FCCP)
Working Together to Strengthen Families

Partnering with Families and Communities to
Raise Healthy Children in a Safe, Caring Environment
and Ensuring A Family for Every Child

Every family hopes their children will be safe, happy and productive members of society. Sometimes, behavior or emotional problems, troubles in school or with the law, or family life in general put kids at risk of ever achieving the life we envision for them.

The way kids get help in times like these has been transformed. Barriers to services are coming down, and families are being given a voice, a choice, and a shared responsibility for changing the course of their children's lives.

The Department recognizes that all families struggle from time to time and need supports they can turn to when they need assistance. These supports range from basic needs to bridging family rifts with the over-arching goal to increase the overall well-being of families.

Our Family Care Community Partnership (FCCPs) agencies, part of DCYF's network of prevention-focused providers, brings community-based services to children and families in order to build a stronger, brighter future. FCCP's focus on your family, and wrap you with the right local services and community programs to build a stronger family.

Each FCCP uses a nationally recognized practice model Wraparound to successfully bring a family to its full potential. Wraparound - a team of professionals, collaborating with a family will develop a personalized, coordinated care plan that Picture of Family
"surrounds" the family with services that address the issues confronting the family - from basic needs, to mental health and behavioral counseling, to healthcare, educational needs and much more.

Picture of Family The family's unique care plan will utilize services from literally hundreds of local professional resources, as well as family, friends, and other "natural Supports" that can help families achieve their goals. The FCCP strives to wrap the family with the supports it needs with the goal of establishing ongoing natural supports from their community and/or family.

The FCCP's also provide supportive services to youth transitioning back to their homes from the Thomas C. Slater Training School for Youth. These youth and their families need appropriate supports to decrease the likelihood of future involvement with the Juvenile Justice system.
