Description: The Department created the Care Management Team (CMT) to partnered with community providers to create a regional based teams to address the needs of children and youth (involved with DCYF) needing specialized home based supports programs and residential treatment if appropriate.
The Team consists of the youth (if appropriate), the youth’s parents, individuals that the family identify as supportive (other relatives, clergy, neighbors, etc), DCYF staff, and a representative from the following organizations; the local Community Mental Health Center, the local Child and Adolescent Social Service Program (CASSP), Placement Solutions, Special Education Department from the local school department, Parent Support Network, and any other members of the community that provide services to the family.
Team meetings are held weekly and take into consideration the following factors when making recommendations; the strengthens of the family and their support systems, how the youth is currently functioning, the level of risk to the youth, to their family and/or to the community, the needs of the child and/or the family and the steps that have been taken to address such needs, recommendations in any reports or evaluations done of the youth and the families wishes.
