Description: It is the belief and practice of the RITS that Transitional Planning commences upon intake. Once adjudicated each resident has an Individual Treatment Plan that is review bimonthly and culminates in a specialized Transitional Planning Meeting. Unit Managers, Clinical Social Workers, Transitional Coordinator, Education staff, Medical personnel, Service Providers, residents, families, advocates and others, as appropriate, are active members of this process.
